Thrive at Home Community Support
Thrive at Home strives to move away from the traditional domiciliary care model.
We provide recovery focused individually tailored support to adults with mental illness in the localities of Swansea and Neath Port Talbot.
Our vision for 'Thrive at Home' is simple, we believe that every person has the right to ‘be their best self and live their best life’, a life which is best lived out in the community and not in hospital.

We know that with the correct level of support, a recovery focused model of care and a sense of purpose through meaningful occupation our services users not only live in the community, but thrive.

As part of our community outreach we run social groups around Swansea and Neath Port Talbot to meet the social and emotional needs of not only our clients but anyone else who wants to join us.

The Thrive at Home team of highly skilled and well-trained Mental Health Recovery Workers is headed up by our extremely experienced Registered Social Care Manager who is also a qualified Mental Health Nurse.
We are able to offer bespoke person centred packages of support to clients both via local authority and outside of the council’s funding framework, to those clients who wish to self-fund or use direct payments to fund their community support.

Support plans are individualised depending on each person’s needs. Some of the common types of support we can provide include but aren’t limited to:

- Medication management or prompting
- Support with shopping
- Support with cooking and nutrition
- Monitoring of mental state
- Companionship
- Support with accessing the local community safely
- Support to attend appointments
- Prompting activities of daily living such as personal care
- Support with light domestic duties
- Social and emotional support
